The link above is the link directly to the form. Yes, it's published, but remember you had me modify the PHP file to make it so that it would work on click instead of on page load? What I didn't understand was how to translate what you said into the HTML that I need to put into the content item...?

Ok, then you need to put that code back, i am not seeing any link with the onclick feature, so if you want to make your popup show you need to put the deleted code back in the php file!
